Ordering Information
Please fill out the coupon below. Include your name, address, city, state, and zip (9 digit if you can). Try to include your CSA ID# from the mailing label on your CSA News. For each item, be sure to include the Catalog #, the title of the product, quantity, price, and total. Total the "total" column, add shipping cost, and give total amount due. Enclose the coupon and a check, made out to CSA, for the proper amount in an envelope addressed to CSA, 22509 State Line Road, Cleveland, MO 64734.
